terms of employment..
Hi! Please note, you are in charge of learning how to be an “employer”, this guide is simply a reminder of what to expect for my hiring process. Thank you!
Contact will take you to a short questionnaire that you can submit and I will reach out within 48-72 hrs with confirmation that I received your email.
Upon contacting you back, we may set up a phone screening to introduce ourselves, discuss cares, discuss a contract, & you can expect to be emailed a contract (for review purposes only).
The phone screening will allow me to take down information on the possible client & what’s expected of me so that those duties can be added to the contract, as well.
Reminder: I ONLY provide Non-medical private care. Please refer here for my, Rates & Services.
Over the phone, we may decide to continue with the process. If applicable, we’ll set up an interview date where I can meet the client (you or someone else), go over the contract in person to discuss questions and possible additions to it, and then discuss taxes and percentages.
The decision is not final on the interview date. I’ll take up to 14 days to decide if this is a good fit for me, and you will have the same 14 days to think about it. Once the 14 days are over and if you have not decided/responded, the hiring process will end.
If we determine that you’ll be my employer, you MUST:
already have an EIN number
Verify work eligibility with Form I-9
Obtain workers compensation insurance
Be registered with the state through WI DOR
All of those must be done before my first shift.
When we set up a start-date, this will include Paid Orientation (30-60 minutes before the first shift begins).
During the orientation, you must provide a W4 form for me to fill out, which I’ve attached here, Forms and Filing information.
W4 form makes me your official employee. (yay!)
During the orientation, I will provide the Contract for us to sign together.
Withholding my taxes are your responsibility as the employer:
Taxes are to be withheld from my earnings each pay period- Federal income (if applicable), State income (if applicable), Social Security, and Medicare.
Federal tax withholdings have to be agreed upon and are based on my Tax Bracket
State tax withholdings for cash wages of $3000 or more in 2026, range from 3.5% tax to 7.65% tax
On WI DOR, you’ll file state taxes quarterly (sometimes monthly) using a WT-6 form and -annually- using a WT-7 form. I’ve attached information on these forms here, Forms and Filing Information.
Social Security & Medicare tax withholdings are paid for by both the Employer and Employee (Both parties pay their own total of 7.65% tax - 6.2% for Social Security and 1.45% for Medicare)
Combined total in Social Security & Medicare tax is 15.3%, every pay period.
When you file your taxes, you’ll use the Schedule H form to find the total household employment tax withheld from the previous year. This form information is here, Forms and Filing information.
